In this web page, we have supplied you a large amount of information, in order help you decide if you want to use our services. We are also happy to answer other questions you have which are not answered by this web page. Please email your questions to pub2@freedonr.com. (You do not need to answer our questions before you email your questions.)

We get many more requests for sperm vials than we have physical resources to satisfy, so we try to select our recipients to maximize benefit to all parties concerned. To help us decide if shipping you our sperm vials is both legal and likely to be most beneficial, we ask that you supply us with answers to the following questions. Please email the answers to us at pub2@freedonr.com. Please answer all of the questions, so that it will not be necessary to spend time exchanging email regarding incomplete answers. Please answer the questions as email text and not as an attached document. We will keep this information strictly confidential.

We will let you know as soon as possible whether or not you qualify.

We will eventually request written proof of the answers to some of these questions.

2) What do you think is important in a child's upbringing and environment? Please describe how you would provide these.

3) Please tell us about the family environment in which you intend to raise your child. Are you married? Are you single and you intend to raise the child as a single mother? Are you in a lesbian relationship? Are you living with a man with whom you are not married?

4) Have you ever tried any sort of fertility treatment before? Artificial insemination? IVF? How soon do you want to start inseminations?

5) How old are you?

6) What is your ethnic background? English? Italian? Chinese? African?

7) In what state/province/country do you live?

8) Please tell us why you want to have a child by artificial insemination and why you are considering the sperm donor you chose.

9) Please describe your educational attainment; specifically, degrees, fields, and schools.

10) Do you have any achievements that you are especially proud of? What are they?

11) Please tell us about your blood family, especially your mother, father, brothers, and sisters. We are especially interested in hearing about unusual accomplishments that make them proud.
